Can anyone help me identify my Stevens Shotgun?

It is a side by side double barrel 16 gauge shotgun. On the left side of the receiver it says "Stevens" with a single duck flying with its wings both pointed down. It is flying over a little patch of grass. On top of the left barrel towards the stock it says "16 GAUGE". On the right side of the receiver it says "J. Stevens Arms Company Chicopee Falls, Mass, U.S.A. -Patented April 20, 1915-

My grandpa gave this to me and all he knows about it is that it is pre WWII. There are no other markings on the gun. It looks like the stock is chipped and cracked and I will probably need a new one so I would like to be sure what model my gun is before I buy a new stock.
